Where is my ancestor buried?

While we regret that historical books containing burial records from Hope and Glenwood Cemeteries are not available to the public due to their delicate condition, the information has been digitized at the Town Hall. Please contact the Cemetery Sexton or Town Clerk to look up burial records.

How do I obtain a copy of my marriage license?

Marriage licenses are only recorded in the Town Clerk's office where the license was originally purchased and at the Department of Health in Albany, NY.


Certified copies of marriage records are only available to the bride, groom or persons with proof of Power of Attorney. In order to obtain a copy of your marriage record from the Perry Town Clerk, you will need to do the following:

  • Complete form DOH-301 (Application for Marriage Record)
  • provide current photo ID
  • have your signature notarized, if you are applying by mail
  • provide a current phone number, if you are applying by mail
  • pay $10, cash or check

Genealogical marriage records are available to direct descendents when the marriage was recorded at least 50 years ago and both the bride and groom are now deceased. In order to obtain a copy of your marriage record from the Perry Town Clerk, you will need to do the following:

  • Complete form DOH-1562 (Application for Genealogical Searches)
  • provide proof of descendency, such as a family tree
  • provide current photo ID
  • have your signature notarized, if you are applying by mail
  • provide a current phone number, if you are applying by mail
  • application fees are based on the time span to search for your record. Basic searches cost $22 for each record and cover a three year search. Rates for more extensive searches are listed on form DOH-1562.

How do I know if I live in the Town or Village of Perry?

There are several ways to determine if you live in the Town or Village:

  • Your house number. If you have a 2 digit house number, you probably live in the Village. If your house has a 4 digit number you probably live in the Town.
  • Tax bills contain a "SWISS" code which gives the location of your property within Wyoming County.
    • 564289 - Town of Perry
    • 564201 - Village of Perry in the Town of Perry
    • 562603 - Village of Perry in the Town of Castile

Where do I obtain a copy of a birth or death record?

Birth and death records are recorded in the town or village where the birth or death occurs. 

Birth and death records from the Town of Perry are available from the Town Registrar at 46 North Main Street, Perry, NY 14530. 585-237-2216
